Requests must be made in writing or electronically to info@mysmile.swiss, including the patient’s full name and the date of birth (and, if applicable, those of the legal representative).
Medical data may also be requested by an insurance company, financial assistance institution (e.g., HG, SPC, SPMi), or competent authorities, provided the request is justified.
Access to the medical file is free of charge.
However, if the request involves a complete report or treatment plan requiring significant administrative work, a fee of up to CHF 300.– may be charged. If such a fee applies, it will be justified and communicated to the patient in advance, allowing them to amend or withdraw their request.
